I have back pain and have to have Therapeutic massage by licensed therapists. It has no sensual connotation. Most therapists are women. Is it allowed to have it for medical reasons. I understand there used to be women for “khidmat”.

You should do your best in looking for a man, if not, and older woman, if not, which is unlikely, then you should take your wife or someone with you to learn how it is done and do it for you. It is not too hard to learn. Besides, the presence of your wife in the room will prevent the possible khalwah (seclusion with a foreign woman).
You must also be honest with yourself in assessing your need for this modality of therapy.
The women for khidmat you are referring to may have been slave girls, who may touch their masters or hired maids, who may not. The ruling of the slave girls doesn’t apply â obviously â to the physical therapists.
